The disease then slowly moves outward to larger areas that involve remembering and reasoning.&amp;nbsp; The new studies indicate it may be possible to bring Alzheimer’s disease to an abrupt halt early on by preventing cell-to-cell transmission, perhaps with an antibody that blocks tau. &lt;/div&gt;&lt;div itemprop="articleBody" style="text-align: justify;"&gt;&lt;br /&gt;&lt;/div&gt;&lt;div itemprop="articleBody" style="text-align: justify;"&gt;The studies, done independently by researchers at Columbia and Harvard, involved genetically engineered mice that could make abnormal human tau proteins, predominantly in the entorhinal (pronounced en-toh-RYE-nal) cortex, a sliver of tissue behind the ears, toward the middle of the brain, where cells first start dying in Alzheimer’s disease. As expected, tau showed up there. And, as also expected, entorhinal cortex cells in the mice started dying, filled with tangled, spaghettilike strands of tau.&amp;nbsp;&lt;/div&gt;&lt;div itemprop="articleBody" style="text-align: justify;"&gt;&lt;br /&gt;&lt;/div&gt;&lt;div itemprop="articleBody" style="text-align: justify;"&gt;&lt;/div&gt;&lt;div itemprop="articleBody" style="text-align: justify;"&gt;Researchers were able to develop genetically engineered mice that expressed abnormal human tau, but only in their entorhinal cortexes. Those mice offered the cleanest way to get an answer.&lt;/div&gt;&lt;div itemprop="articleBody" style="text-align: justify;"&gt;&lt;br /&gt;&lt;/div&gt;&lt;div itemprop="articleBody" style="text-align: justify;"&gt;Over the next two years, the cell death and destruction spread outward to other cells along the same network. Since those other cells could not make human tau, the only way they could get the protein was by transmission from nerve cell to nerve cell. Researchers knew that something set off Alzheimer’s disease. The most likely candidate is a protein known as beta amyloid, which accumulates in the brain of Alzheimer’s patients, forming hard, barnaclelike plaques. But beta amyloid is very different from tau. It is secreted and clumps outside cells. An 85-year-old has a 75 percent chance of living another three years, but only a one in four chance of surviving for 10. Which category a particular old person falls into has much to do with the medical problems he or she has, or doesn’t have, and with his or her ability to function.&lt;/div&gt;&lt;div style="text-align: justify;"&gt;&lt;br /&gt;&lt;/div&gt;&lt;div style="text-align: justify;"&gt;When the odds are that they have only a few remaining years, should doctors discuss that with them? These issues were discussed in a recent article in the New England Journal of Medicine, Dr. Smith, a palliative care specialist at the University of California, San Francisco suggested &lt;a href="http://www.nejm.org/doi/full/10.1056/NEJMp1109990"&gt;offering to discuss “overall prognosis,”&lt;/a&gt; doctorspeak for probable life expectancy and the likelihood of death, with patients who &lt;i&gt;don’t&lt;/i&gt; have terminal illnesses. Congress would establish an insurance exchange for Medicare beneficiaries. Private plans would compete with the traditional Medicare program and would have to provide benefits of the same or greater value. The average approval rate is around 60%.&lt;/div&gt;&lt;div style="text-align: justify;"&gt;&lt;br /&gt;&lt;/div&gt;&lt;div style="text-align: justify;"&gt;A draft of the study is due in August and the final recommendations are to be released in November. The recommendations won't be binding, but they could serve as a blueprint for changes by either the SSA or Congress.&lt;/div&gt;&lt;div style="text-align: justify;"&gt;&lt;br /&gt;&lt;/div&gt;&lt;div style="text-align: justify;"&gt;Another focus of the study will be why federal courts are overturning many of the decisions that Social Security judges make when they deny benefits. My opinion about why the federal courts are overturning more cases appealed to the Courts is that the Appeals Council has been significantly expanded in an effort to decrease the the backlog of cases and .less qualified people are reviewing decisions. There was a story in the Wall Street Journal where an unnamed source indicated this was coming. This will discourage attorneys from taking certain claimant's cases just before the hearing. There are ALJ's that certainly have their bias in favor or against certain claimants based on the nature of their disabilities or past history. We all wish all of our clients were Mother Theresa but there are lot of nonfunctional people out there that have been victims of abuse, had criminal, mental health problems or have had substance abuse problems in their past. These claims are not settled as you must convince the ALJ that your client is disabled. William Bunn, 47 years old, a truck driver from Peoria, Ill., found his disability claim rejected, in part, on the recommendation of a retired pediatrician. Mr. Bunn was diagnosed with small-fiber neuropathy in 2009, a type of nerve disorder that primarily affects older people. Mr. Bunn, who began suffering from pain and numbness in his legs, said he couldn't drive a truck with his condition and quit his job. His application was supported by two private doctors. But it was rejected after two reviews by the Illinois Bureau of Disability Determination Services, one of which was performed by the pediatrician. His appeal took more than two years. Overall use of psychiatric medications among adults grew 22% from 2001 to 2010. The new figures, released Wednesday, are based on prescription-drug pharmacy claims of two million U.S. insured adults and children reported by Medco Health Solutions Inc., a pharmacy-benefit manager.&lt;/span&gt;&lt;/div&gt;&lt;div style="text-align: justify;"&gt;&lt;br /&gt;&lt;/div&gt;&lt;div style="text-align: justify;"&gt;Psychiatric medications are among the most widely prescribed and biggest-selling class of drugs in the U.S. In 2010, Americans spent $16.1 billion on antipsychotics to treat depression, bipolar disorder and schizophrenia, $11.6 billion on antidepressants and $7.2 billion on treatment for ADHD, according to IMS Health, which tracks prescription-drug sales.&lt;/div&gt;&lt;div style="text-align: justify;"&gt;&lt;br /&gt;&lt;/div&gt;&lt;div style="text-align: justify;"&gt;In the elderly, the use of anti-anxiety drugs, particularly ones that stay in the body for a longer period of time like Valium, is a major worry, but there has been relatively little in the way of research on their use in this population, according to Dr. Olfson. It was designed to expand options for people who require long-term help. No cases were closed in the SSA's Boston and Denver regions that day and the Seattle region closed just one case.&lt;/span&gt;&lt;/div&gt;&lt;div style="text-align: justify;"&gt;&lt;br /&gt;&lt;/div&gt;&lt;div style="text-align: justify;"&gt;&lt;span style="font-size: small;"&gt;It is believed that under political pressure to clear the growing backlog of applications, SSA leaders set numerical goals for judges and field offices in order to expedite cases. Each entered the hospital as an emergency patient, usually after a fall, but remained on &lt;span class="yshortcuts cs4-visible" id="lw_1320354370_3"&gt;observation status&lt;/span&gt; for days of full hospital service.&lt;/div&gt;&lt;div style="text-align: justify;"&gt;&lt;/div&gt;&lt;div id="yui_3_3_0_1_1320422207365419" style="text-align: justify;"&gt;None received hospital coverage under Medicare Part A for their stays. Instead they were relegated to the Part B section of the federal program that covers visits to doctors' offices and other outpatient facilities.&lt;/div&gt;&lt;div id="yui_3_3_0_1_1320422207365420" style="text-align: justify;"&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; As a result, they and their families incurred medical charges as high as $30,000 for skilled nursing care, drugs and other costs that Medicare does not cover unless a patient has been admitted to a hospital for at least three days.&lt;/div&gt;&lt;div style="text-align: justify;"&gt;&lt;/div&gt;&lt;div id="yui_3_3_0_1_1320422207365428" style="text-align: justify;"&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; The policy is meant to protect hospitals from Medicare penalties for admissions made in error. But the incidence of observation status has increased sharply in recent years with the advent of federal healthcare reform and heightened scrutiny of Medicare spending, according to Medicare patient advocates.&lt;/div&gt;&lt;div style="text-align: justify;"&gt;&lt;/div&gt;&lt;div id="yui_3_3_0_1_1320422207365550" style="text-align: justify;"&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; The result can be tens of thousands of dollars in medical costs for beneficiaries, who do not qualify for Medicare hospital coverage while on observation status."This causes severe financial problems for beneficiaries and their families," said &lt;span class="yshortcuts cs4-visible" id="lw_1320354370_5"&gt;Judith Stein&lt;/span&gt;, executive director of the Center for Medicare Advocacy. Because there &lt;i&gt;will&lt;/i&gt; be a cost-of-living increase for Social Security recipients in 2012, the Part B premium will increase, but only by $3.50 – from $96.40 in 2011 to $99.90 in 2012.&lt;a href="mailbox:///C%7C/Documents%20and%20Settings/Bill%20Wombacher/Application%20Data/Thunderbird/Profiles/h7fo5rot.default/Mail/Local%20Folders/Inbox?number=2300526126#_ftn1" name="_ftnref1" title=""&gt;[1]&lt;/a&gt;&amp;nbsp;For those individuals who &lt;i&gt;did &lt;/i&gt;have Part B premium increases in 2010 and 2011, the premium will actually decrease by $15.10 in 2012, from $115 to $99.90.&lt;br /&gt;&lt;br /&gt;                                            The Part B premium reductions are a result of slower Part B growth due in part to health care reform. Upon exhaustion,  dedicated revenue will be able to pay 90% of costs for the  hospital-insurance program.&lt;br /&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; For the first time since 1983, Social Security spending was greater than  non-interest income in 2010. It is estimated to affect up to 5 million U.S. adults, most  commonly middle-aged women.The  precise cause of fibromyalgia is unknown. There are no physical markers,  like inflammation or tissue damage in the painful areas -- but some  researchers believe the disorder involves problems in how the brain  processes pain signals. Standard  treatments include painkillers, antidepressants, cognitive-behavioral  therapy and exercise therapy. One  reason, some researchers suspect, may be because standard treatments do  not specifically address the role psychological stress and emotions can  play in triggering pain.&lt;br /&gt;&lt;span id="articleText"&gt;The study, published in the journal Pain,  looked at the effects of so-called mindfulness-based stress reduction --  a technique developed by researchers at the University of Massachusetts  in 1979 that combines mindfulness meditation and gentle yoga postures.The  technique is now available throughout the world -- in the form of an  eight-week program of classes -- to help people manage general stress or  health problems, including chronic pain.&lt;br /&gt;&lt;span id="articleText"&gt;For the new study, researchers led by Dr. Stefan Schmidt, of the University Medical Center in Freiburg, &lt;a href="http://www.reuters.com/places/germany" title="Full coverage of Germany"&gt;Germany&lt;/a&gt;, tested the program's effects among 177 women with fibromyalgia.They  found that women assigned to the mindfulness program showed no greater  gains in health-related quality of life than those assigned to a waiting  list for treatment.That meant no significant improvements in either physical symptoms or emotional well-being.&lt;br /&gt;&lt;br /&gt;Amy Norton writes about his for Reuters online service. The  Treasury expects the shift to save Social Security $1 billion over the  next 10 years.&lt;/div&gt;&lt;div style="text-align: justify;"&gt;Under the final plan, anyone applying for benefits on or after May 1,  will receive their payments electronically. People who already receive  paper checks will need to switch to direct deposit by March 1, 2013.&amp;nbsp;&amp;nbsp;&lt;/div&gt;&lt;div style="text-align: justify;"&gt;&lt;br /&gt;&lt;/div&gt;&lt;div style="text-align: justify;"&gt;The government's new electronic-payment requirement has some exceptions.  It has waived, for instance, for check-receiving recipients who are age  90 or older or approaching 90. Specifically, benefit recipients born  before May 1, 1921, who would like to keep receiving paper checks will  be able to do so under the final rule. In addition, the Treasury is touting its prepaid card, the Direct  Express Debit MasterCard, for benefit recipients who don't choose direct  deposit. She had clients sign a power of attorney, giving  her access to their funds—and spent more than $3 million of their money  on her own expenses, including clothing and vacations, according to  court documents.&amp;nbsp;&lt;/div&gt;&lt;div style="text-align: justify;"&gt;&lt;br /&gt;&lt;/div&gt;&lt;div style="text-align: justify;"&gt;"That's a definite new trend," says Denise Voigt Crawford, the Texas  securities commissioner. "We're seeing more cases of older people  ripping off other older people. Someone joked that seniors ripping off  their peers is becoming 'the new retirement plan.'" Some older financial advisers use their age as a selling point, telling  clients they understand the challenges that older investors face. In  many instances, say prosecutors, unscrupulous advisers also tout their  professional designations, or credentials, as further evidence of their  expertise.
